Rural Telehealth Sounds Perfect. Is It?
Use the downloadable checklist to ask how a program or clinic handles connection failures, privacy, urgent needs, in-person care, and local follow-up.
Start with the patient’s connection.
HHS rural telehealth guidance recommends checking internet access, devices, private space, and a backup way to connect. A video visit only helps if the patient can join and has a clear fallback when it fails.
Name what cannot happen on video.
Ask who decides when an in-person exam, local testing, referral, or urgent evaluation is needed. Do not assume that telehealth replaces those services. The checklist asks who arranges each handoff and how the patient learns the next step.
Make access more than broadband.
Ask how the team addresses privacy, interpreters, accessibility needs, and a safe place for the visit. HHS lists rural access barriers and resources, but a specific clinic still needs its own workable process.
Follow one case through to completion.
In a program visit, request a supervised example from scheduling through the visit, a failed connection or in-person need, documentation, and follow-up. Mark a step “unknown” if the program cannot identify who owns it.

A program or clinic should be able to walk through a real workflow for failed connections, urgent needs, local services, and follow-up. If ownership of any step is unclear, ask for a supervised case example before assuming the training covers rural telehealth well.
Make the guide specific to you.
- Connection: Does the patient have a device, usable connection, private location, and a documented backup if video fails?
- Safety: Who triages urgent concerns, and what is the escalation path to local in-person or emergency care?
- Access: How are interpreters, disability accommodations, privacy, and a patient’s preferred contact method handled?
- Local services: Who arranges exams, testing, referrals, or prescriptions that cannot be completed virtually?
- Follow-up: Who communicates results, documents the handoff, and checks that the next step happened?

Hi [program contact], I am comparing NP clinical training. Could you walk me through one supervised rural telehealth case, including a failed connection, an in-person referral, privacy, and who follows up with the patient? Thank you.
