A clinical supervisor is strongest when the program permits that relationship and the person directly evaluated your work. Useful evidence can include assessment, prioritization, escalation, communication, teamwork, reliability, response to feedback, leadership, and safe care in the relevant population or acuity.
Provide accurate dates, unit context, responsibilities, and two or three moments the supervisor can choose to discuss. Do not provide patient-identifying information or draft praise for the writer to copy.

What Should an Academic Recommender Document?
An academic recommender is strongest when the person taught, supervised, or evaluated recent work that resembles graduate study. Evidence may include analytical writing, clinical reasoning, research, statistics, group work, response to critique, consistency, and completion of difficult material.
Choose someone who can describe performance, not merely confirm enrollment. If the program requests a recent faculty member and you have been out of school, ask whether a clinical instructor, preceptor, educator, or graduate-course faculty member qualifies. Obtain the answer before sending the request.
When Should an NP or Physician Write the Letter?
An NP or physician should write when that professional relationship is allowed and the person directly observed relevant work. The credential itself is not the evidence.
Clarify whether the person was your supervisor, collaborating clinician, preceptor, colleague, or mentor. Match the application label to the real relationship. A provider who barely knows you is usually less useful than an allowed supervisor who can document sustained judgment and growth.

How Do You Ask and Prepare Recommenders Ethically?
An ethical request asks whether the person can write a strong, accurate recommendation by the required date. Give the person a graceful way to decline.
Send a packet with:

- Current resume or CV and relevant transcript context.
- Your program-specific goals and personal-statement themes.
